Please contact your insurance company to check if your policy provides benefits for Allergy/Immunology. It is helpful for you to know what your insurance will cover and how much your estimated portion will be when coming to the clinic. Ask your insurance company if you have a deductible to meet, and how much of that has been met. There will be an office visit charge for New patients and Established patients in addition to the allergy testing codes below.

Allergy Testing Codes:
CPT 95004- Skin Scratch Test.
Depending upon how many scratch tests are performed we recommend getting a quote for 30 to 65, but this will vary per individual.
CPT 95024- Intradermal Test
Depending upon how many intradermal tests are performed we recommend getting a quote for 10 to 15, but this will vary per individual.
Immunotherapy Codes – (for patients who decide to go on allergy injections/shots)
CPT 95165-Allergy Extract (refill every 4 to 6 months)
Depending upon what is recommended for each patient, we recommend getting a quote for 30 to 60 units. This fee will cover 4 to 6 months of allergy extract.
CPT 95115 Allergy Injection (1)
OR
CPT 95117 Allergy Injections (more than 1)
Allergy Injections can be given once to twice weekly depending on the individualized plan with the physician.
